It really works great. Very pleased, but I would like to see it updated to the latest version of WordPress (4.0), so we know it will continue to work. Also, it would be useful if a check box could be added. In Europe, we are required to place checkboxes for accepting terms and conditions. But it is so hard to find plugins that have them! Thank you!
