Works fine except I identified a small bug where the form prints errors if the user doesn’t fill out a rating but still submits a comment. Easily fixed by adding curly brackets to the conditional on line 99.

This plugin does exactly what i need. I just wanted to put a rating field to the comments on a simple way. The documentation can be better for people that don’t know much about theme files.
